How does PACE work?<\/h2>\n

PACE covers Medicare and Medicaid-covered care and services, and other services that the PACE team of health care professionals decide are necessary to improve and maintain your health. This includes medication, as well as any other assessed need: physical, emotional and social care, like doctor or health care provider visits, transportation, home care, hospital visits, and even nursing home stays when assessed as a need. HopeWest PACE serves seniors from all walks of life. Our program can enroll individuals who meet the following eligibility requirements:<\/p>\n
\n- Are 55 years and older<\/li>\n
- Live in the HopeWest PACE specific service area in Mesa County<\/li>\n
- Meet the state\u2019s nursing facility level of care criteria<\/li>\n
- Live safely in the community with PACE services<\/li>\n<\/ul>\n
