PACE Care Team
HopeWest PACE participants have access to a variety of specialists during the course of their participation in the program. At the core of the HopeWest PACE program is the Interdisciplinary Team (IDT). This team of specialists communicates with each other regularly and has access to the participant’s medical records and wishes for care. They determine your plan of care along with you and your loved ones. When you enroll in HopeWest PACE, you will meet with all of the following professionals:
- Primary Care Physician
- Center Director
- Clinic Nurse
- Recreation Therapist
- Home Care Nurse
- Social Worker (MSW)
- Dietician
- Home Health Aide
- Occupational Therapist
- Physical Therapist
- Transportation Coordinator
