Who We Serve

at HopeWest PACE

The HopeWest PACE program provides seniors in our community the care they need to remain living safely in the community.

HopeWest PACE serves seniors from all walks of life. Our program can enroll individuals who meet the following eligibility requirements:

  • Are 55 years and older
  • Live in the HopeWest PACE specific service area in Mesa County
  • Meet the state’s nursing facility level of care criteria
  • Live safely in the community with PACE services
