for HopeWest PACE Participants
Delivering all needed medical and supportive services, HopeWest PACE provides the entire continuum of care and services to seniors with chronic care needs while maintaining independence in their homes for as long as possible.
Because HopeWest PACE is responsible for all your care, all services must be authorized by the PACE team, except emergency services. PACE participants may be fully and personally liable for the costs of unauthorized or “out-of-PACE” program agreement services.
These are some of the services included in the PACE program and fully covered when authorized by the interdisciplinary team that includes nurses, doctors, therapists, social workers, and others:
- Adult primary care (including doctor and recreational therapy nursing services)
- Dentistry
- Home care
- Hospital care
- Laboratory/x-ray services
- Meals
- Medical specialty services
- Nursing home care
- Nutritional counseling
- Occupational therapy
- Physical therapy
- Prescription drugs
- Preventive care
- Social services, including caregiver training, support groups, and respite care
- Social work counseling
- Transportation to the PACE Center for activities and/or medical appointments in the PACE Clinic, and medical appointments in the community.
A hot lunch and snacks are available daily Monday-Friday at the HopeWest PACE Center catered by Spoons bistro & bakery. Lunch is served in a comfortable group setting, and special dietary needs are easily accommodated by our nutrition experts.
