What is PACE?

What is PACE?

PACE is a comprehensive health care program created over 30 years ago. PACE provides an integrated approach with medical and supportive services designed to help individuals meet their health care needs in the community.

With PACE, a team of health care professionals works with each individual to make sure needed services are coordinated. PACE programs get to know their participants and design services to meet each person’s unique needs.

How does PACE work?

PACE covers Medicare and Medicaid-covered care and services, and other services that the PACE team of health care professionals decide are necessary to improve and maintain your health. This includes medication, as well as any other assessed need: physical, emotional and social care, like doctor or health care provider visits, transportation, home care, hospital visits, and even nursing home stays when assessed as a need. Individuals can disenroll from a PACE program at any time by notifying the PACE team.

Who can enroll?

HopeWest PACE serves seniors from all walks of life. Our program can enroll individuals who meet the following eligibility requirements:

  • Are 55 years and older
  • Live in the HopeWest PACE specific service area in Mesa County
  • Meet the state’s nursing facility level of care criteria
  • Live safely in the community with PACE services

HopeWest PACE does not discriminate in the delivery of PACE services based on race, ethnicity, national origin, religion, sex, sexual orientation, age, mental or physical disabilities or source of payment.

How is PACE paid for?

HopeWest PACE accepts Medicaid, Medicare, and private payment.

  • If you have Medicaid only, there is no premium.
  • If you have Medicare and Medicaid, there is
    no premium.
  • If you have Medicare but not Medicaid, there is a monthly premium.
  • If you do not qualify for Medicare or Medicaid, there is a monthly premium.

HopeWest PACE participants must receive all needed healthcare, including primary care and special physician services (other than emergency services) from HopeWest PACE contracted providers. HopeWest PACE participants may be fully and personally liable for costs of any unauthorized out-of-network services.

HopeWest PACE participants may disenroll at any time.
